Breast fat grafting revision surgery

When a previous surgery went wrong
This applies when the implants have not taken root, when you wish to remove and replace them, or when the left and right sides do not align. The criteria for evaluating revision surgery are different from those for the initial surgery. If you don't first figure out why it happened, the same thing will happen again.

I had breast fat grafting, but the grafts didn't take root. Can I have it done again?

It is often possible. However, you must first determine why the graft failed to take root.

Three causes: ① Fat purity during the harvesting and processing stage ② Excessive injection into a single site ③ Post-operative weight changes and management

The next plan varies completely depending on the cause. We proceed after identifying the cause by measuring the previous surgical records and current condition in 3D.

Limit If you reinsert it without knowing the cause, the same thing will happen again. If there is no record, check the current condition first using imaging.

When is the best time for revision surgery? Is there a golden time?

You can accurately measure the current condition only after the point where engraftment stabilizes has passed.

Usually stabilizes after 6 months; absorption is in progress before then.

If it is too early, absorption is still in progress, so it is impossible to determine what remains, and the tissue has not yet recovered.

Limit Conversely, there is no reason to delay it excessively. The exact timing depends on the previous surgical method and course.

I want to remove my implants and replace them with fat grafting. Is it possible to do it in one go?

Whether to perform the procedure in one go or in stages depends on the space where the implant was located and the condition of the tissue.

When implants are removed, a space remains; inserting a large amount before the tissue has settled is detrimental to engraftment. If capsular contracture has occurred, capsule treatment must also be considered.

Limit If you aim to finish it all at once, you will suffer a loss in graft survival. It is often more stable to break it down into smaller steps.

Is it possible to add only fat grafting without removing the implants?

It is possible. However, the amount that can be injected is limited due to different blood flow conditions around the implant.

There is a hybrid method that adds fat to the deficient areas while leaving the implant in place. The decision is made based on the current position, size, and capsule condition of the implant.

Limit Since there are limitations on how much can be placed on each layer, it may not fit if you expect a large volume.

I had revision surgery because the saline solution ruptured, and I also had fat grafting because of my uneven breasts, but one side is still smaller.

It is common for the condition of the left and right tissues to differ after undergoing multiple surgeries.

In this case, you do not put the same amount on the left and right sides. Designed differently to produce the same resultIt must be done. After numerically confirming the current left-right volume difference using 3D measurement, the side with less tissue margin is approached in stages.

Limit Perfect symmetry is not the goal. Graft survival rates may differ between the left and right sides, so differences may be visible during the course of treatment.

I have capsular contracture. Can it be resolved with fat grafting?

No. Capsular contracture is not resolved by adding more fat.

Capsular contracture is a condition in which the capsule surrounding the implant thickens and contracts. Treatment of the capsule comes first, and only then is the method for filling the volume determined.

Limit The options vary depending on the grade and tissue condition. Confirmation through imaging and physical examination is required.

Is the incision for revision surgery made in the same spot as the first surgery?

Since autologous fat grafting is injected using an ultra-fine cannula, the incision area itself is small.

If implant removal is performed, the existing incision is typically used, and the procedure is planned to minimize scarring.

Limit It may vary depending on the past scar condition and the extent of capsule treatment.

I had a failed surgery at another hospital; is it possible to have a re-operation here?

Where you had surgery previously is not the criterion for judgment. The current tissue condition and the previous surgical method are the criteria.

If possible, bringing your previous surgery records (surgery name, materials used, injection volume, and progress) will make the diagnosis of the cause much more accurate.

Limit It can proceed even without records, but it relies heavily on imaging tests and physical examinations.

The shape of my breast, reconstructed after breast cancer surgery, became unsightly over time.

The assessment of correction after reconstruction varies significantly depending on the treatment history and tissue condition.

Whether or not radiation therapy has been performed is particularly important. Tissues that have received radiation have different blood flow and elasticity, so the conditions for engraftment differ.

Limit In this case, we cannot provide an answer via online consultation. Please bring your previous surgery or treatment records with you when you visit for medical treatment.

Surgical outcomes vary from person to person, and all medical procedures carry the possibility of side effects and complications. An in-person consultation is required to assess the individual's condition.
